Online Triage Form
You can contact us using our online form for admin queries such as, fit notes, repeat prescriptions, referral updates etc, as well as non-urgent medical queries.

Please be aware that this e-mail inbox is not continuously monitored and therefore should not be used for any urgent or medical queries.
In particular it should not be used for any of the following:
- To book an appointment or cancel an appointment: appointments must be made via the telephone, the NHS app or using our online triage form. Due to high demand, we understand that there is a wait but thank you for your patience.
- Sick notes: these can be requested via our online triage form or via reception.

Who To Contact When We Are Closed
If you need urgent medical help when we are closed – please call 111.
NHS 111 can help if you have an urgent medical problem and you’re not sure what to do.

Disabled Access
There is good access for the disabled to both surgeries via a ramp and wide doors to allow for wheelchair access.
All of our consultation rooms are accessible easily on the ground floor, or via a lift to the first floor at Queen Square. We do not allow mobility scooters in the lift at Queen Square, so please inform us prior to your appointment if you need an appointment on the ground floor or you may ask at reception to borrow our wheelchair if required.
Disabled parking is available on Queen Street to the rear of the surgery; you will then be required to make your way around Queen Square and to the front entrance.
At Caton Health Centre there is disabled parking within the carpark.
Transport
Both sites are easily accessible via public transport with bus stops directly outside both sites.
