Registering with the Practice

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

Registering

You can register by completing the online form:

Alternatively you may print off the registration forms, fill them out and bring in to the practice, or visit the practice to fill in a hard copy. Paper copies also require a signature to be accepted.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

The registration process can take up to 5 days if submitted online, or up to 2 weeks if submitted in paper form; if you are on regular medication please ensure you have an adequate supply to cover you for this period.

We recommend that new patients undertake a health check with a practice nurse.

Please note, you will be registered under Queen Square Medical Practice, with Caton Health Centre as our branch site providing basic services. This means that you can request appointments at Caton Health Centre, however it is not guaranteed one will be available. Procedures like minor operations and urgent care amongst others are available at Lancaster only. It’s important to note that there are no permanent GPs stationed at Caton; GPs may work one or two sessions weekly or may not be available there at all based on their other commitments at the main site in Lancaster.

Temporary patients

If you are not registered with a GP you will be unable to see one unless you have a serious emergency. In most other cases, calling 111 is a better option.

If you are not registered with a GP but need treatment at a GP Surgery you will need to complete a temporary registration form in the surgery.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
