If you change your details please let our team know by completing a form in the surgery, or filling in the online form below. If you move outside the practice area you may need to find a doctor in your new area. Check our practice boundary here. There are also instructions on what to do when you move to a new area on the back of your NHS medical card.
Please note, that for a change of name or date of birth, PCSE require that we see evidence of such, i.e. passport, driving license, deed poll documentation. We are also unable to change your registered gender via this form; this will require a routine GP appointment to discuss in the first instance.
